Are you a new patient?
When the scheduling form asks whether your child is a new patient, please answer “Yes” if your child has not previously been seen by HouseCall For Kids — even if your child has been seen by one of our providers at another practice. Our system can only locate existing records for patients who were previously treated under HouseCall For Kids. If your child has not been seen here before, selecting “New Patient” ensures your information is entered correctly.

- • Standard 15-minute visits: $150 | Extended 30-minute visits: $250
